Blackpool forms part of Lancashire and South Cumbia Hep C Operational Delivery Network,  and has one of the highest HCV prevalence rates, recent  report ‘HCV commissioning template for prevalence and treatment’ demonstrates  1397 people living in Blackpool with HCV, of these 80.2% have been referred to HCV specialist services, only 36% commencing treatment. To tackle this issue we have developed a Hep C community clinic and outreach worker role, due to the very early success of this pilot we have sought funding to secure an 18 month post.


Role and Responsibilities

 The role entails working specifically with the hardest to reach HCV positive individuals to re-engage them into HCV treatment, whilst offering wrap around support and navigation into other support services.

·      Locating and motivating into treatment

·      Follow-up to clinic visits, including scheduling all appointments and follow-up on clinician referrals, appointment reminders

·      Transport to and support within appointments

·      Compose weekly email updates regarding upcoming clinical visits

·      Provide feedback to HCV ODN and line manager

·      Attend the Hepatitis C (HCV) patients’ clinic visits during clinic sessions with patients.

·      Provide support for patients and address their questions and concerns;

·      Clarify information and answer questions regarding disease pathology, treatment eligibility and evaluation, hepatitis C treatment and medications, and managing co-morbidities

·      Assist patients on hepatitis C treatment by providing support for patients while on hepatitis C treatment, by providing assistance in managing side effects

·      Develop a HCV peer programme and group

·      Fully monitor and track performance of the project with line manager

·      Assist with research and grant activities as needed

·      Literature review, recruitment, consenting patients and conducting assessment interviews


What you will need

 ·      A relevant degree in health and social care or comparable qualification such as DANOS level3 drug and alcohol practitioner, other training and qualifications of a similar background will be considered

·      Either lived or work experience in this area

·      Experience and knowledge of working with vulnerable and hard to reach adults

·      Excellent motivational interviewing techniques

·      Excellent knowledge of Hep C virus, new medications and treatment, and eligibility criteria

·      Excellent organisational and time management skills

·      Knowledge and experience of coordinating and monitoring a project from start to end

·      Excellent IT/report writing and presentation skills

·      MUST hold a current driving licence and a vehicle, the role requires transporting patients to and from appointments where needed
